Job Summary
Conduct adult and geriatric health assessments in members' homes or Skilled Nursing Facilities, reviewing medical history, medication adherence, and social environments while administering point-of-care and lab-based testing. Collaborate with physicians, case managers, and caregivers to manage referrals, provide patient education, and ensure clear documentation per Matrix guidelines. Perform visits lasting up to one hour, traveling to locations outside your home state if needed, with flexible scheduling from Sunday through Saturday. Complete monthly averages of 20 to 100 assessments to earn between $1,800 and $11,500, including mileage reimbursement and access to continuing education.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice
- Prescription authority or the ability to obtain prescriptive authority
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Bodily Injury: $100,000 per person/$300,000 per incident
- Property Damage: $50,000
- Collision and comprehensive deductibles: $1,000 or less
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills

- 1 year of NP experience preferred
