Job Summary
Conduct adult and geriatric health assessments at members' residences or skilled nursing facilities, reviewing medical history, medication adherence, and social environment to identify care gaps. Administer and order point-of-care and lab-based testing as needed while collaborating with physicians, case managers, and caregivers on patient education and follow-up. Provide clear documentation according to Matrix guidelines and protocols during visits lasting up to one hour. Schedule availability between 8 am and 7 pm, Sunday through Saturday, with flexible day, evening, or weekend options. Earn $1,800 to $11,500 monthly based on assessment volume, with opportunities to transition to full-time roles and receive additional licensure support.
Required Qualifications
- Master's Degree or commensurate experience
- Satisfactory completion of NP licensure
- Current NP licensure in state of practice
- Prescription authority or the ability to obtain prescriptive authority as needed
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Bodily Injury: $100,000 per person/$300,000 per incident
- Property Damage: $50,000
- Collision and comprehensive deductibles: $1,000 or less
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills

- 1 year of NP experience preferred
