Job Summary
Conduct adult and geriatric health assessments at members' residences or skilled nursing facilities, reviewing medical history, medication adherence, and social risks to identify care gaps. Administer and order point-of-care and lab-based testing as needed while collaborating with physicians, case managers, and caregivers on patient education and follow-up. Provide clear documentation and referrals according to Matrix guidelines during visits lasting up to one hour. Schedule availability from Sunday through Saturday between 8 am and 7 pm, with pay based on assessment completion rates ranging from $90 to $115 per visit.
Required Qualifications
- Master's Degree OR commensurate experience and satisfactory completion of NP or PA licensure
- Current NP or PA licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority
- Board certified by the NCCPA, AANP, ANCC or the AACCN in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
- Current BLS, ACLS or CPR certification
- Valid state driver's license and proof of adequate automobile insurance coverage for the state of residence
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
Desired Qualifications
