Job Summary
Conduct comprehensive appraisals of patients' health status by taking histories and performing physical examinations at patients' homes in western Chicago suburbs or assisted living facilities. Provide direct clinical care, prescribe medications, order laboratory tests and imaging, and evaluate patients to determine further health care needs. Deliver comprehensive teaching to patients and families, document all care in medical records, and refer to other disciplines as appropriate. Maintain certifications including BLS and ACLS, participate in quality improvement and research initiatives, and serve as a mentor to graduate students. Confers with collaborating physicians and participates in departmental committees. Requires a Master's degree, current Illinois APRN or PA license, and clinical privileges. Schedule includes Monday through Friday with call coverage after clinic hours and four evenings monthly.
Required Qualifications
- Master's degree from an accredited school/college of nursing OR master's degree from an accredited Physician Assistant Program
- Current license as a Registered Nurse in the State of Illinois
- Current license as an Advanced Practice Registered Nurse in the State of Illinois
- Certification as an Advanced Practice Registered Nurse by a national certifying body as approved by the Illinois Department of Regulation APRN Board
- Master's degree from an accredited Physician Assistant Program
- Current license as a Physician Assistant in the State of Illinois
- Privileged and credentialed through the Medical Staff Office (MSO)
- Basic Life Support for Healthcare Providers (issued by the American Heart Association) certification
- Advanced Cardiovascular Life Support (ACLS) certification
- Controlled substance license and DEA registration
