Job Summary
Conduct comprehensive assessments, develop treatment plans, and provide ongoing care management for residents in skilled nursing and senior living facilities. Perform routine rounds to assess conditions, adjust treatment plans, and address emergent health concerns while collaborating with facility staff, families, and other providers. Maintain accurate EMR documentation, participate in quality improvement initiatives, and educate residents and families on disease prevention and wellness. This role offers weekend rounding with a sign-on bonus and supports Curana's mission to improve health outcomes across 1,500+ communities.
Required Qualifications
- Valid nurse practitioner or physician assistant license in state of operations
- Current board certification in your field
- Masters Degree in Nursing or Physician Assistant Studies
- Must possess excellent written and oral communication skills
- Must possess traits of punctuality, organizational skills, and ability to function within a team
- Valid driver's license
- Current auto insurance
- Acceptable driving record
