Job Summary
Provide Primary Care and/or Consultative Professional Care to elderly, frail, and seriously ill patients in home and facility settings, in collaboration with physicians according to a Collaboration Arrangement. Develop and implement treatment plans, perform medical evaluations, interpret laboratory tests and diagnostics, prescribe medications within prescriptive authority, and coordinate with the multidisciplinary team to improve quality of life and reduce unnecessary ER/hospital visits. Responsibilities include communicating significant findings to clinicians and families, facilitating continuity of care, and supporting population health initiatives within a home health and hospice-focused practice. Location is Saginaw, MI with service areas including Gaylord, Boyne City, Grayling, and surrounding communities.
Required Qualifications
- Master’s or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant
- Current State License as a Nurse Practitioner or Physician Assistant
- Sit, stand, bend, lift, and move intermittently
- Must be able to lift at least 35lbs and bear the weight of an average adult effectively
- Current and valid driver’s license
- Reliable transportation and valid automobile insurance coverage
- Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in skilled nursing facility settings
- Demonstrates skill in practice, ability to cope with family emotional stress, and tolerance of individual lifestyles
- Ability to handle multiple priorities, documentation requirements, and deadlines
