Job Summary
Provide adult/geriatric health assessments during home visits or in SNFs, reviewing medical history, diagnoses, treatment plans, and patient education; order and perform point-of-care and lab testing; collaborate with physicians, case managers, and other care team members; support ongoing referrals and follow-up; travel up to 80-90% across the state and possibly neighboring states; base salary range $105,500–$168,400 with sign-on bonus up to $7,500; requires NP/PA licensure, board certification, and driving to patient residences; full-time position with opportunities for licensure expansion across states.
Required Qualifications
- Master’s Degree OR commensurate NP/PA licensure and experience
- Current NP/PA licensure in state of practice with prescriptive authority or ability to obtain
- NCCPA, AANP, AACN or ANCC board certification in appropriate specialty
- Current BLS, ACLS or CPR certification
- 1 year of NP or PA experience
- Ability to travel up to 80-90% of the time across the assigned state and possibly neighboring states
- Valid state driver’s license and auto insurance
- Strong computer skills and familiarity with medical record software
- Excellent verbal and written communication skills
