Job Summary
Nurse Practitioner or Physician Assistant – Full Time State Traveler. Provide in-home health assessments and care coordination across members’ residences (homes or Skilled Nursing Facilities) with ongoing collaboration with PCPs and care teams. Requires Master’s-level education or equivalent NP/PA licensure, current state licensure with prescriptive authority, board certification in Adult/Adult Gerontology/Family/EM/AC care, BLS/ACLS, and 1 year of NP/PA experience. Must be able to travel 80-90% of the time across the assigned state and possibly neighboring states; sign-on bonus of $7,500 and incentive opportunities included.
Required Qualifications
- Master's degree OR commensurate NP/PA licensure experience
- Current NP/PA licensure in state of practice with prescriptive authority or ability to obtain
- NCCPA/AANP/AACN/ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency or Acute Care
- Current BLS, ACLS or CPR certification
- 1 year of NP or PA experience
- Ability to travel up to 80-90% of the time including overnight travel and weekends across the assigned state and possibly neighboring states
- Valid state driver’s license
- Automobile insurance coverage for the state of residence (or ability to obtain)
- Mileage reimbursement provided
