Job Summary
Conduct adult and geriatric health assessments at member residences or skilled nursing facilities, reviewing medical history, medication adherence, and social risks to identify care gaps. Administer point-of-care and lab-based testing as needed while collaborating with physicians, case managers, and caregivers to manage referrals and patient education. Perform documentation and follow-up in accordance with Matrix guidelines, supporting the Comprehensive Health Assessment protocol across the nation. Full-time Monday through Friday schedule with 80-90% travel requirements including overnight trips.
Required Qualifications
- Master's Degree OR commensurate experience and satisfactory completion of NP or PA licensure
- Current NP/PA licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- NCCPA, AANP, AACN or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency or Acute Care specialty
- Current BLS, ACLS or CPR certification
- 1 year of NP or PA experience
- Able to travel up to 80-90% of the time for extended periods of time to include overnight travel and weekends across the assigned state and possibly neighboring states
- Valid state driver's license and proof of adequate automobile insurance coverage for the state of residence
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
