Job Summary
Conduct adult and geriatric health assessments at member residences or skilled nursing facilities, reviewing medical history, medication adherence, and social environments to identify care gaps. Administer point-of-care and lab-based testing, manage physician referrals, and collaborate with primary care physicians, case managers, and caregivers on patient education and follow-up. Provide comprehensive documentation according to Matrix guidelines while traveling up to 80-90% of the time across the state and neighboring regions. Full-time role requires Master's degree or commensurate experience with current NP or PA licensure, including prescription authority, along with BLS, ACLS, or CPR certification.
Required Qualifications
- Master's Degree OR commensurate experience and satisfactory completion of NP or PA licensure
- Current NP/PA licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed
- NCCPA, AANP, AACN or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency or Acute Care specialty
- Current BLS, ACLS or CPR certification
- 1 year of NP or PA experience
- Able to travel up to 80-90% of the time for extended periods of time to include overnight travel and weekends across the assigned state and possibly neighboring states
- Valid state driver's license for the state of residence
- Automobile insurance coverage requiring for the state of residence (or ability to obtain)
- Bodily Injury: $100,000 per person/$300,000 per incident
- Property Damage: $50,000
- Collision and comprehensive deductibles: $1,000 or less
- Strong computer skills and familiarity with employee health/medical record software
- Excellent verbal and written communication skills
