Job Summary
Conduct daily patient rounds in an Inpatient Rehabilitation Facility (IRF) setting, performing focused assessments, monitoring rehabilitation progress, and addressing acute and subacute medical needs for patients undergoing intensive recovery. Manage comorbidities and complications common in the IRF population, including post-stroke recovery, orthopedic post-op care, traumatic brain injury, cardiac conditions, and neurological disorders. Collaborate closely with physiatrists, therapy teams, and interdisciplinary staff to develop coordinated care plans and support optimal functional recovery and safe discharge planning. Write and manage orders for medications, diagnostics, therapies, and consults while responding promptly to nursing and therapy team communications. Evaluate and manage acute changes in patient condition, initiate appropriate interventions, and determine when escalation or transfer is warranted. Maintain thorough documentation in the EMR consistent with IRF regulatory standards and CMS compliance guidelines. Communicate regularly with patients and families regarding medical status, goals of care, and discharge expectations. Participate in quality improvement initiatives focused on reducing hospital transfers and improving functional outcomes. Schedule includes Mon-fri rounding with one weekend per month required; this role requires travel to and from one or more assigned facilities.
Required Qualifications
- Valid nurse practitioner or physician assistant license in state of operations
- Current board certification in your field
- Masters Degree in Nursing or Physician Assistant Studies
- Must possess excellent written and oral communication skills
- Must possess traits of punctuality, organizational skills, and ability to function within a team
- Valid driver's license
- Current auto insurance
- Acceptable driving record
