Job Summary
Conduct comprehensive assessments, develop treatment plans, and manage chronic conditions for residents in skilled nursing and senior living facilities. Perform regular rounds to monitor health status, adjust care protocols, and address emergent concerns. Collaborate with facility staff, families, and other providers to coordinate comprehensive care, while educating residents and families on disease prevention and wellness. Maintain accurate documentation within the EMR system and participate in quality improvement initiatives to optimize patient outcomes.
Required Qualifications
- Valid nurse practitioner or physician assistant license in state of operations
- Current board certification in your field
- Masters Degree in Nursing or Physician Assistant Studies
- Must possess excellent written and oral communication skills
- Must possess traits of punctuality, organizational skills, and ability to function within a team
- Valid driver's license
- Current auto insurance
- Acceptable driving record
