Job Summary
Manage a patient panel with a broad array of patient needs including acute, chronic, and well visits; provide in-office and testing-center care, with occasional tele-health visits; participate in ongoing clinical rounds and daily huddles with in-office and virtual teams; perform in-office procedures and be willing to obtain additional state licensure/credentialing for virtual primary care in additional states; fully integrate into a team-focused, human-centered primary care model in a San Francisco, CA setting with relocation support potential.
Required Qualifications
- Completed an accredited NP or PA program with a national certification
- State licensed in California, obtained by start date
- In the past 5 years, practiced as an Advanced Practitioner for at least: 2 years in an outpatient primary care setting, OR 1 year outpatient primary care plus 1 year fellowship or 1+ year urgent care
- Experience in LARC (IUD + Nexplanon) insertion and colposcopy procedures is a plus
