Job Summary
Provide primary care to a patient panel with a broad array of patient needs; conduct acute, chronic, and well visits; treat patients in-office or in testing centers and conduct occasional tele-health visits; participate in weekly Clinical Rounds and daily huddles with in-office and virtual clinical teams; utilize clinical training to perform in-office procedures; pursue additional state licensure for One Medical virtual primary care in additional states; based in Fairfield County, CT; base salary range $146,400 to $155,530 with potential bonuses; relocation may be available; comprehensive benefits.
Required Qualifications
- Completed an accredited NP or PA program with a national certification
- In the past 5 years, practiced as an Advanced Practitioner for at least: 2 years in an outpatient primary care setting, OR 1 year in an outpatient primary care setting, coupled with either a 1 year primary care fellowship or 1+ year in an urgent care setting
- State licensed in CT, obtained by your One Medical start date
