Job Summary
Provide comprehensive care to seniors following clinical standards while collaborating daily with physicians, pharmacists, and behavioral health specialists to ensure coordinated, high-quality outcomes. Manage referrals, hospital/SNF coordination, DME, and home health services, and document care with support from quality-based coders. Participate in on-call rotation and support growth plans at your center within a value-based care model that prioritizes spending more time with patients for personalized physical, emotional, and social wellness. Work on-site in Orlando, FL, as part of a collaborative team delivering proactive, preventive care including wellness visits and chronic condition management.
Required Qualifications
- Master's degree in nursing with board certification
- Active, unrestricted NP, or PA license in the state of Florida
- Experience in value-based care, primary care or Medicare Advantage settings
- Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
- Familiarity with HEDIS quality indicators and Medicare guidelines
