Job Summary
Remote Nurse Practitioner or Physician Assistant to join the Virtual Health Assessment team. Responsible for preventative care screenings, medication reconciliation, diagnosing medical conditions, providing health coaching, referrals and treatment as appropriate. Outreaches to chronically ill members based on historical and suspected health information, engages members who may have lower PCP visit probability, and identifies opportunities to improve access to care. Cross-trains into urgent care and/or virtual primary care service lines, interacts with members virtually, and ensures compliance with applicable laws and regulations. Requires PA or DNP, FNP, ENP with board certification, Spanish bilingual proficiency, and multi-state licensure potential; 3+ years of outpatient Family Medicine, Internal Medicine, or Primary Care experience.
Required Qualifications
- PA or DNP, FNP, ENP from accredited program
- Board Certification (AANP, ANCC, or NCCPA)
- Compact RN (NP only)
- Experience with Clinical Documentation Improvement and/or managing HEDIS quality measures
- Physician Assistant: Must be fully licensed in Georgia and one of the following states: Florida, Texas, or Ohio
- Nurse Practitioner: Must be fully licensed in at least 2 of the following states: Florida, Texas, Ohio, New Jersey
- Bilingual - Spanish fluency (read, write, & speak)
- Ability to obtain licensure in additional states
- 3+ years experience in outpatient Family Medicine, Internal Medicine, or Primary Care
