Job Summary
Deliver mobile on-site primary care services to vulnerable patients in assisted living and senior communities, managing complex scenarios with multiple co-morbid conditions. Educate patients and families on self-care, disease management, and end-of-life care while collaborating daily with facility staff and support teams. Operate autonomously using the Elation EMR, adhering to quality measures and maintaining strict confidentiality. Requires valid licensure, at least one year of primary care experience, AZ fingerprint clearance, and current CPR certification. Contract position with full-time hours in West Valley, AZ, offering $70/HR for up to 6 months.
Required Qualifications
- Must have at least one (1) year of primary care experience
- Valid Licensure as a Nurse Practitioner or Physician Assistant in State of Practice
- Ability & Desire to work independently, and drive between communities
- Valid AZ Fingerprint Clearance Card
- Current CPR Certification & Proof of Negative TB Test
