Job Summary
Evaluate, diagnose, and manage acute, chronic, and episodic cardiovascular conditions for patients in Skilled Nursing, Long-Term Care, and Post-Acute Care facilities across the Peoria area. Order, interpret, and communicate laboratory studies and imaging; prescribe medications within scope; and develop individualized treatment plans using evidence-based clinical practice. Coordinate care with physicians, nursing staff, and interdisciplinary teams while maintaining accurate clinical documentation and rounding at assigned sites. Build strong relationships with facility leadership and represent Alteas Health with professionalism and clinical excellence. Full-time Monday through Friday daytime schedule with no regularly scheduled weekends or traditional on-call responsibilities.
Required Qualifications
- Current, unrestricted Nurse Practitioner or Physician Assistant license in the applicable state(s) of practice
- Current national certification appropriate to licensure
- Current Basic Life Support (BLS) certification
- Minimum six (6) months of Nurse Practitioner or Physician Assistant clinical experience
- Valid driver's license with reliable transportation
- Ability to travel daily throughout an assigned territory
- Strong clinical judgment, communication, organizational, and critical thinking skills
- Experience utilizing Electronic Medical Records (EMR)
Desired Qualifications
- Two (2) or more years of Nurse Practitioner or Physician Assistant experience
- Experience in Skilled Nursing, Long-Term Care, Post-Acute Care, Internal Medicine, Cardiology, Pulmonology, Hospital Medicine, or Geriatrics
- Experience caring for medically complex patient populations
