Job Summary
Manage patients receiving hospital-level care in the home setting by obtaining comprehensive histories, performing physical examinations, and formulating evidence-based treatment plans. Order, interpret, and perform diagnostic and therapeutic procedures within scope of practice while monitoring progress and evaluating intervention effectiveness. Coordinate consultations, referrals, and transitions of care alongside interdisciplinary teams to ensure seamless delivery. Provide education, counseling, and support to patients and families while maintaining prescribing authority and clinical privileges per Arizona standards. Serve as an advocate for quality, safety, and continuous improvement within the Advanced Care at Home program, promoting awareness of the model among patients, families, and healthcare colleagues.
