Nurse Practitioner- Oakbrook Clinic

Co-manage a defined panel of geriatric patients alongside a lead physician, performing preventative screenings and leading discussions on long-term goals of care. Review and update patient medication lists, order laboratory and diagnostic procedures, and maintain medical records accurately. Provide secondary support across the care continuum in post-acute settings such as AL/IL and SNFs, while managing complex cases including polypharmacy, frailty, dementia, and chronic disease optimization. Maintain a high level of independent clinical decision-making in a fast-paced environment with direct mentorship from a fellowship-trained geriatrician. Full-time role with a mix of clinic days and facility rounding.
