Job Summary
In-House Nurse Practitioner at a Skilled Nursing Facility within Trio Health Partners, providing primary internal medicine services to geriatric residents. Lead clinical care alongside an interdisciplinary team, conduct new admission evaluations and monthly stability visits, perform comprehensive physical examinations, prescribe medications, develop patient care policies, coordinate advanced care planning and discharge visits, and ensure timely documentation and communication of assessments and treatment plans; target of at least 12 patient visits per day with bonuses for surpassing this target.
Required Qualifications
- Active Illinois Nurse Practitioner License (unencumbered)
- Master's Degree in Nursing from an accredited program
