Job Summary
Provide comprehensive medical care to patients in nursing homes and home-based settings, performing history and physical examinations, routine follow-ups, and urgent evaluations. Diagnose and manage acute and chronic medical conditions for elderly and medically complex patients, ordering and interpreting diagnostic tests and labs. Develop individualized treatment and care plans while coordinating care with primary care physicians, specialists, nursing staff, and caregivers. Educate patients and families on disease management, medications, and preventive care, maintaining accurate documentation in the electronic health record. Participate in care coordination, quality improvement initiatives, and population health efforts. Full-time role in Manhattan, NY; new graduates with geriatrics interest encouraged.
Required Qualifications
- Graduate of an accredited Nurse Practitioner program (MSN or DNP)
- Active or eligible New York State Nurse Practitioner license
- National certification (FNP, AGNP, or GNP)
