Job Summary
Provide primary care to high acuity patients in their home Monday through Friday. Deliver patient-centered, physician-driven care under an integrated team-based model for individuals with complex healthcare needs. Assist with appointments in select markets and utilize company resources including a vehicle with driver where available. Maintain licensure, DEA eligibility, and active CPR/BLS certification while focusing on quality outcomes rather than volume. No hospital rounds or holidays; minimal call responsibilities.
Required Qualifications
- Licensed as a NP in the State where HarmonyCares Medical Group office located
- Federal DEA License/eligibility
- Active CPR/BLS Certification
- Board Certification
- Must maintain a valid driver's license and good driving record
Desired Qualifications
- Geriatric training/experience preferred
