Job Summary
Provide primary care to high acuity patients in their home Monday through Friday, managing chronic conditions without weekend or evening shifts. Assist with appointments in select markets and utilize a company car with driver where available. Maintain a valid driver's license and good driving record while adhering to the integrated, team-based, physician-driven model. This role focuses on patient-centered care rather than volume, supporting the mission to bring personalized healthcare to patients with difficult access.
Required Qualifications
- Licensed as a NP in the State where HarmonyCares Medical Group office located
- Federal DEA License/eligibility
- Active CPR/BLS Certification
- Board Certification
- Must maintain a valid driver's license and good driving record
Desired Qualifications
- Geriatric training/experience preferred
