Job Summary
Provide primary care to high acuity patients in their home Monday through Friday. Assist with appointments in select markets and utilize a company car with driver where available. Deliver patient-centered care under an integrated, team-based, physician-driven model for people with complex healthcare needs. Maintain a valid driver's license and good driving record while holding an active NP license and Federal DEA eligibility. No holidays or hospital rounds are required, and call responsibilities are minimal.
Required Qualifications
- Licensed as a NP in the State where HarmonyCares Medical Group office located
- Federal DEA License/eligibility
- Active CPR/BLS Certification
- Board Certification
- Must maintain a valid driver's license and good driving record
Desired Qualifications
- Geriatric training/experience preferred
