Nurse Practitioner (NP) Primary Care

Provide primary care to high acuity patients in their home Monday through Friday, focusing on geriatric chronic condition management within a value-based care model. Spend 30 to 45 minutes with each patient during the workday, completing charting tasks using Athena and AI Scribe tools. The role supports an integrated, physician-driven model for patients with complex healthcare needs who have difficulty accessing traditional care. No weekends, evenings, or holidays are required, with minimal call responsibilities. A company car with a driver is available in select markets.
