Job Summary
Provide primary care to high acuity patients in their home Monday through Friday, focusing on geriatric chronic condition management under a value-based care model. Deliver services to patients in Kent, Federal Way, Auburn, and Renton areas while adhering to a three-day schedule per week (Monday, Tuesday, and Friday). Work within an integrated, physician-driven team model to support patients with complex healthcare needs who have difficulty accessing traditional care. Maintain a valid driver's license and good driving record for all appointments.
Required Qualifications
- Licensed as a NP in the State where HarmonyCares Medical Group office located
- Federal DEA License/eligibility
- Active CPR/BLS Certification
- Must maintain a valid driver's license and good driving record
Desired Qualifications
- Geriatric training/experience preferred
- Board Certification
