Job Summary
Provide primary care to high acuity patients in their home Monday through Friday. Assist with appointments in select markets and utilize a company car with driver where available. Deliver patient-centered care under an integrated, physician-driven model for individuals with complex healthcare needs who have difficulty accessing traditional services. Maintain licensure in the state of practice, hold a valid DEA license, and possess geriatric training or experience.
Required Qualifications
- Licensed as a NP in the State where HarmonyCares Medical Group office located
- Federal DEA License/eligibility
- Active CPR/BLS Certification
- Board Certification
- Must maintain a valid driver's license and good driving record
