Job Summary
Nurse Practitioner delivers compassionate medical care to residents in sub-acute, long-term, rehab, and outpatient settings, conducts examinations, and collaborates with the health care team to update and modify care plans; documents care appropriately and educates residents, families, and staff. Reports to Medical Director or supervising Attending Physician. Responsibilities include coordinating with the health care team, providing quality clinical leadership during night/weekend shifts, and leveraging EMR systems to maintain accurate charts. This Part-Time role requires NY NP licensure with prescriptive privileges and NYS Board Certification, with 2+ years in sub-acute/long-term care and geriatric experience preferred. The position is based in Queens, NY, with shifts from 11:00pm to 7:00am on weekends. Required Qualifications
- Two (2) or more years of experience in a sub-acute or long term care facility
- Graduate of an accredited nurse practitioner program
- Active license as an NP in New York State, with prescriptive privileges
- NYS Board Certified is required
- Proficient in SigmaCare or another EMR (Electronic Medical Record) software
- Prior experience with the geriatric population is a plus
