Job Summary
Deliver person-centered care using evidence-based practices while conducting assessments, developing treatment plans, and managing follow-up care in skilled nursing facilities. Collaborate with interdisciplinary facility teams and educate patients, families, and staff. Complete timely documentation within 24 hours and ensure compliance with state regulations. Required Qualifications
- Active license as a Nurse Practitioner (NP) or Physician Assistant
