Job Summary
Deliver person-centered care as a Nurse Practitioner or Physician Assistant in skilled nursing facility settings, including conducting assessments, developing and managing treatment plans, and providing follow-up care. Collaborate with interdisciplinary facility teams, educate patients, families, and staff, and complete timely documentation (within 24 hours). Ensure compliance with state regulations in SNF settings. Requires active NP/PA licensure, 2+ years post-certification experience, and proficiency with EHRs and clinical technology; strong communication and collaborative skills. Required Qualifications
- Active license as a Nurse Practitioner (NP) or Physician Assistant (PA)
