Job Summary
Nurse Practitioner or Physician Assistant role in a skilled nursing facility setting with Impact Health PC in Norway, ME. Deliver person-centered, evidence-based care in collaboration with interdisciplinary facility teams. Responsibilities include conducting assessments, developing treatment plans, managing follow-up care, educating patients and families, and ensuring timely documentation within 24 hours while complying with state SNF regulations. Required Qualifications
- Active license as a Nurse Practitioner (NP) or Physician Assistant (PA)
- Minimum 2 years of post-certification experience
- Experience in SNFs, hospitalist roles, emergency departments, or ICU/SICU preferred
- Proficient with EHRs and clinical technology
- Strong communicator with a collaborative mindset
