Job Summary
Deliver person-centered care using evidence-based practices while conducting assessments, developing treatment plans, and managing follow-up care in skilled nursing facilities. Collaborate with interdisciplinary facility teams to educate patients, families, and staff, ensuring all documentation is completed within 24 hours and state regulations are met. Provide independent care with access to physician-level consults for ongoing clinical support. This role offers a flexible Monday through Friday schedule with no nights, weekends, or holidays, and includes comprehensive health benefits, malpractice insurance, and license reimbursement.
Required Qualifications
- Active license as a Nurse Practitioner (NP) or Physician Assistant
- Minimum 2 years of post-certification experience
- Proficient with EHRs and clinical technology
