Job Summary
Travel to patients' homes in and around Chase City, VA to perform comprehensive, non-invasive physical assessments. Review medical history, ensure accurate documentation, conduct medical reconciliation for patient safety, and provide basic education on wellness and preventative healthcare. This role focuses on completing in-home Health Risk Assessments for Medicare and Medicaid members without treating or prescribing. Flexible scheduling allows for part-time work with travel reimbursement and comprehensive home assessment training.
Required Qualifications
- Master's Degree OR commensurate experience and satisfactory completion of NP/PA license
- Active State NP/PA license
- Active National board certification
- Current BLS certification
Desired Qualifications
- 12 months of experience as a paid advanced clinical provider
