Job Summary
Lead clinical care in multiple skilled nursing facilities as a Board-Certified Nurse Practitioner. Provide routine and acute medical care with clinical autonomy, conduct comprehensive assessments, manage complex chronic conditions, oversee personalized care plans, and collaborate with facility nursing leadership and attending physicians to ensure integrated care. Drive quality improvement initiatives focused on reducing preventable hospital readmissions. Enjoy a four-day, four-ten-hour-shift schedule aimed at work-life balance. Eligible for premium benefits including medical/dental coverage, 401k, malpractice coverage, and full license/certification/DEA renewals reimbursement. Required Qualifications
- Active and unrestricted Nurse Practitioner License in the state of Ohio
- Active Nurse Practitioner Certification
- Board-certified NP
- Experience with skilled nursing/rehab preferred
- Ability to work autonomously in SNF settings
- New graduates considered with preference to 3+ years RN experience for this setting
