Job Summary
Provide primary care to high acuity patients in their home Monday through Friday. Deliver personalized, quality-based healthcare under an integrated, team-based, physician-driven model. Assist with appointments in select markets and utilize company vehicles with drivers where available. Maintain a valid driver's license and good driving record. This role focuses on patient care rather than volume, supporting patients who have difficulty accessing traditional care.
Required Qualifications
- Licensed as a NP in the State where HarmonyCares Medical Group office located
- Federal DEA License/eligibility
- Active CPR/BLS Certification
- Board Certification
- Must maintain a valid driver's license and good driving record
Desired Qualifications
- Geriatric training/experience preferred
