Job Summary
Provide comprehensive medical care, palliate, and manage terminal illnesses under the supervision of a collaborating physician. Evaluate health histories, diagnose conditions, and develop personalized care plans while consulting with the attending physician, medical director, and interdisciplinary team. Coordinate patient oversight, conduct eligibility assessments for certification and recertification, and deliver education on medical, psychological, and spiritual concerns to patients and families. Maintain strict compliance with state Nurse Practice Act, Medicare/Medicaid, and federal regulations. Deliver compassionate end-of-life care in the Red Bank Area on a per diem basis, with compensation of $150 per visit.
Required Qualifications
- Master's degree
- Current unencumbered license in the State of Practice as a Registered Nurse
- Current unencumbered license in the State of Practice as a Nurse Practitioner
- Current driver's license
- Vehicle insurance
- Access to dependable transportation
- Possesses and maintains CPR certification
Desired Qualifications
- Minimum of one (1) year hospice or palliative care experience as an RN or NP
- Certification in a specialist area (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
- Excellent verbal and written communication abilities
- Background in EMR
