Job Summary
Nurse Practitioner in an autonomous Community-based Advanced Illness Management (AIM) program in Southern Illinois. Schedule grants Mon-Fri, 8a-5p with NO nights/weekends/call. Mileage reimbursement available. Practice with virtual support from a collaborating physician and peer APPs; comprehensive financial package and benefits starting within 30 days. Qualifications include graduation from an accredited NP program and IL NP licensure; experience in palliative care, hospice, or geriatrics. Location: Mount Vernon, IL, with SSM Health systemic support and an emphasis on compassionate, autonomous patient care.
Required Qualifications

- Current NP licensure in Illinois
