Job Summary

Nurse Practitioner Hospice and Palliative care role provides medical care, palliation, and management of terminal illness under physician supervision. Responsible for history and physical examinations, developing individualized plans of care, prescriptive treatments, hospice eligibility assessments, and coordination with the hospice interdisciplinary team. Duties include delivering patient care in home/community settings, completing face-to-face visits for hospice benefit periods, documenting clinical findings, ensuring HIPAA compliance, and participating in quality improvement. Requires Master's degree and NP experience in hospice/palliative care, state NP licensure and DEA where applicable, and CHPN or equivalent certification preferred. Strong collaboration with the interdisciplinary team, patients, families, and external care providers; proficient in clinical documentation and communication; capable of teaching and counseling patients and families. Ensures compliance with Medicare/Medicaid rules and HIPAA; engages in ongoing professional development and program growth.

Required Qualifications

  • Graduation from an accredited School of Nursing and an accredited Nurse Practitioner Program
  • Current DEA license, if applicable to state regulations
  • Current nursing licensure in State and CPR certification
  • Master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
  • Accreditation from an approved certifying body for advanced practice nursing as required by State
  • Certification in a specialist area preferred; e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics
  • Excellent observation, verbal and written communication skills; problem solving skills; nursing skills per competency checklist
  • Prolonged or considerable walking or standing; ability to lift, position and / or transfer patients; lift supplies and equipment; considerable reaching, stooping, bending, kneeling and/or crouching; visual acuity and hearing to perform required nursing skills
  • Must be a licensed driver with automobile and insurance in accordance with state/organization requirements
