Job Summary
Perform comprehensive patient assessments, diagnose acute and chronic conditions, and develop individualized treatment plans during home and facility visits. Prescribe medications, manage ongoing chronic illnesses, and educate patients on disease management and preventive care strategies. Collaborate with physicians and specialists to coordinate referrals and ensure continuity of care while maintaining accurate documentation in EMR/EHR systems. Conduct follow-up visits to monitor progress and adjust care plans as needed. This role offers a competitive salary of $130,000 – $200,000 annually with comprehensive benefits, including EMR training and professional development opportunities.
Required Qualifications
- Master's Degree from an accredited Nurse Practitioner program
- Active New York State NP license
- National Board Certification (AANP, ANCC, or ONCC)
- Strong clinical judgment
- Communication skills
- Ability to work independently
