Job Summary
Provide primary care to high acuity patients in their home Monday through Friday. Deliver patient-centered care under an integrated, team-based, physician-driven model for people with complex healthcare needs. Assist with appointments in select markets where available. Maintain a valid driver's license and good driving record. Requires state NP licensure, Federal DEA eligibility, and active CPR/BLS certification. Geriatric training and board certification are preferred. No holidays or hospital rounds. Minimal call responsibilities.
Required Qualifications
- Licensed as a NP in the State where HarmonyCares Medical Group office located
- Federal DEA License/eligibility
- Active CPR/BLS Certification
- Board Certification
- Must maintain a valid driver's license and good driving record
