Nurse Practitioner (NP) – Full-Time – In-Home Health Risk Assessments (HRA) – $7,500 Sign-On Bonus

Conduct comprehensive adult and geriatric health risk assessments and screenings for Medicare and Medicaid members in their homes and residential settings. Review medical history, perform medication reconciliation, identify chronic conditions, and provide patient education during 60-minute visits. Document findings in the EMR and collaborate with primary care physicians when needed. This role focuses on preventive care and early detection rather than managing ongoing treatment plans or patient panels. Full-time Monday–Friday schedule with no on-call requirements.
