Job Summary
Conduct comprehensive health assessments, prescribe medications, and manage acute and chronic illnesses as a central provider of care within a multidisciplinary team. Formulate individualized treatment plans, maintain accurate electronic medical records, and participate in interdisciplinary meetings to enhance patient outcomes. Provide full scope primary health care services including health promotion, reproductive care, and mental health support while developing a patient panel of 650. Engage in ongoing professional development and deliver patient education on disease prevention and self-management strategies. Serve the Indigenous population holistically in a community setting featuring abundant natural and oceanfront beauty.
Required Qualifications
- Master's degree in nursing with specialization as a Nurse Practitioner
- Current registration and licensure with the British Columbia College of Nurses and Midwives (BCCNM)
- Certification as a Nurse Practitioner (NP) from a recognized regulatory body
- Proficiency in utilizing electronic medical records (EMR) and other healthcare technologies
- Strong interpersonal and communication skills, with the ability to collaborate effectively within an interdisciplinary team
- Commitment to providing patient-centred care and promoting health equity within the community
