Job Summary
Monogram Health seeks skilled Nurse Practitioners to join an integrated Care Team focused on improving quality of life for patients with chronic health issues. Responsibilities include conducting comprehensive assessments, maintaining accurate patient records, coordinating care, and delivering evidence-based healthcare. Candidates must have an active RN and NP or PA license, board certification, DEA certification, and 2 years of direct patient care experience. This role emphasizes patient education and effective communication within the care team.
Required Qualifications
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
- Current and unrestricted DEA certificate
- Basic Life Support (BLS) certification
- 2 or more years of direct patient care
Desired Qualifications
- Bilingual (English/Spanish)
- Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home
- Managed Care/IPA/Health Plan experience
Additional Requirements
- Ability to work without direct supervision and practice autonomously
- Access to transportation, a valid driver's license, and car insurance
