Job Summary
Conduct in-home Health Risk Assessments for Medicare Advantage members, including medical history, lifestyle, and basic physical or mental evaluations. Provide preventive care education and document visits using a company-provided iPad. Travel across Essex, Suffolk, and Middlesex counties with reliable personal transportation. Complete credentialing, training, and onboarding within 10-12 days to begin field work immediately. Earn $125 per visit with additional incentives for no-shows and diagnostic-only encounters. This role offers total autonomy, flexible scheduling, and mileage reimbursement after 20 miles per day.
Required Qualifications
- Nurse Practitioner (NP) license
- No Physician Assistant (PA) license
- No Pediatric NP specialty
- No PMHNP specialty
- No Women's Health specialty
- 40+ hours per month availability
- Immediate start availability
- Ability to complete credentialing paperwork, training, and onboarding within 10-12 days
- Ability to start field-based work immediately after onboarding
- Reliable personal transportation
- Willingness to cover 3 counties (Essex, Suffolk, Middlesex)
