Job Summary
As a Nurse Practitioner at Monogram Health, you will provide personalized medical care primarily for patients with chronic kidney disease (CKD) and end-stage renal disease (ESRD/ESKD). Responsibilities include conducting comprehensive wellness examinations, coordinating patient care, maintaining accurate patient records, and prescribing medications. You will educate patients on health benefits, perform assessments, and collaborate with care teams to enhance patient outcomes while adhering to federal and state health regulations.
Required Qualifications
- Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
- Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
- Current and unrestricted DEA certificate
- 2 or more years of direct patient care required
Desired Qualifications
- Managed Care/IPA/Health Plan experience
- Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home
