Job Summary
Perform routine and follow-up medical evaluations while managing chronic and acute conditions. Collaborate with physicians and interdisciplinary teams to develop individualized treatment plans, review labs and medications, and communicate with patients and nursing staff. Focus on preventing unnecessary hospitalizations and delivering compassionate, patient-centered care, utilizing medical scribes to complete documentation. Work Monday through Friday with no nights or weekends, supported by collaborative physician leadership and modern technology.
Required Qualifications
- Active New York State Nurse Practitioner License
- DEA Registration (or ability to obtain)
- Strong clinical assessment and communication skills
- Experience in: Family Medicine, Internal Medicine, Geriatrics, Skilled Nursing, Rehabilitation, Long-Term Care, Sub-Acute Care
