Nurse Practitioner

Advanced practice provider (NP or PA) with geriatrics focus sought to deliver primary care within long-term care facilities and assisted living communities. Responsibilities include assessing patients’ medical and mental health needs, creating and adjusting plans of care, prescribing medications and ordering labs/diagnostic procedures, monitoring treatment response, ensuring compliance with local/state/federal regulations, coordinating with RN case managers as part of an integrated care team, and training facility staff as needed. The role requires independent clinical judgement with supervision by a physician, and use of Electronic Health Records. Experience in geriatric populations and a licensed APRN or PA program graduate is required; MS Office proficiency is also noted.
