Nurse Practitioner

Advanced practice provider (NP or PA) with experience in primary care or internal medicine, preferably geriatrics, to deliver coordinated, proactive healthcare for long-term care residents. Responsibilities include assessing medical and mental health needs, developing care plans, prescribing medications and ordering labs, monitoring patient response, and collaborating with RN case managers and facility leadership to implement a national care model in compliance with regulations. Requires independent clinical judgement, ability to work with supervising physicians, and proficiency with EHR and basic Microsoft Office tools; duties emphasize improving quality of life and reducing hospital visits within nursing homes or assisted living settings.
