Job Summary
Nurse Practitioner (Adult/Geriatric) needed to join an interdisciplinary care team serving frail seniors in Brooklyn, OH and Elyria, OH. The NP will act as primary care provider in collaboration with on-site physicians to deliver comprehensive health services, including assessment, diagnosis, treatment, and prescribing. Responsibilities include managing acute and chronic illness, promoting wellness and preventative care, and following patients across outpatient settings (skilled and long-term nursing facilities, assisted living, group homes, and participant homes) with IDT support. The role also involves serving as a clinical resource and preceptor for clinical orientation, assisting in program and participant outcomes data collection and performance improvement initiatives, and participating in an on-call rotation (from home). Must maintain licensure, certification, prescriptive authority, and complete continuing education; CPR/BLS required. Bilingual English/Spanish candidates are encouraged. Required Qualifications
- Masters prepared nurse with certification as a Family Nurse Practitioner or Adult/Gerontology Nurse Practitioner holding active, unrestricted license in the state of Ohio, required
- Minimum of two years direct care experience working with frail, older adults in community, long term, skilled care, or hospital, required
- Minimum of two years advanced practice nursing experience, required
- Previous experience in interdisciplinary care setting, desirable
- Bilingual candidates (English/Spanish) encouraged to apply
- CPR (BLS) certification, required
